Should healthy individuals wear continuous glucose monitors?

Continuous glucose monitors (CGMs) have emerged as a revolutionary tool in diabetes management, prompting interest from both medical professionals and individuals seeking to optimise their health. 

These small, wearable devices offer real-time glucose readings throughout the day and night, providing a more comprehensive picture of blood sugar fluctuations than traditional finger-prick tests. However, many healthy non-diabetic people are seen wearing these devices, which can have negative results.

I even used one for 2 weeks before realising that other than freaking out anxiously 30-60 mins after every meal, I didn’t learn anything useful that has not been common general nutritional knowledge for at least several decades. ”

Dr Jagadish J Hiremath, public health intellectual, mentions, “Continuous Glucose Monitors (CGMs) provide real-time glucose readings by measuring interstitial fluid glucose levels. They are generally accurate but can have slight delays compared to blood glucose levels.”

CGMs have been found to be reliable, he states, though occasional discrepancies with blood glucose levels can occur, particularly during rapid changes in glucose levels.

Dr Hiremath lists the following benefits of using CGMs:

Reduced Hypoglycemia: Studies, such as those in The Lancet Diabetes & Endocrinology, have shown that CGMs can significantly reduce the incidence of hypoglycemia by alerting users to dropping glucose levels before they reach dangerous lows.

Improved Glycemic Control: Research from the American Diabetes Association has noted that regular use of CGMs has been associated with better glycemic control, reducing HbA1c levels.

Convenience and Comfort: Unlike traditional methods, CGMs eliminate the need for frequent finger pricks, making glucose monitoring less invasive and more convenient.

Should healthy people use CGMs?

CGMs can educate the general public about how different foods impact blood sugar levels, fostering better dietary choices, says Dr Hiremath. “They may help identify prediabetes or undiagnosed diabetes by showcasing abnormal glucose spikes, prompting individuals to seek medical advice. Healthy people can learn how their bodies react to specific foods, promoting more personalised and effective dietary plans.”

However, there are several downsides to this. These include:

Misinterpretation of Data: Non-diabetics might not understand the context of glucose spikes, leading to unnecessary worry or dietary restrictions. Blood sugar levels naturally fluctuate, and without proper knowledge, these readings can be misinterpreted.

Overemphasis on Short-Term Spikes: Focusing on immediate glucose responses rather than long-term health metrics might lead to an unbalanced perspective on nutrition and health.

Potential for Disordered Eating: Constant monitoring can lead to obsessive behaviours and anxiety around food choices, potentially contributing to disordered eating patterns.

Cost and Accessibility: CGMs can be expensive and may not be justifiable for individuals without a medical need, diverting resources from those who need them most.

Dr Hiremath adds, “While CGMs provide valuable insights for diabetics, their utility for the general population is more nuanced. They can promote health awareness and better food choices but also risk fostering anxiety and misinformation.” 

He continues, “Influencers showcasing their glucose levels can raise awareness, but it’s crucial for viewers to understand the broader context and consult healthcare professionals for personalised advice. The trend can be beneficial if balanced with accurate information and mindful use.”
